Phyllis Ann (Small) Sullivan

Phyllis Ann Sullivan, 88, of Mountain Home, Arkansas passed away at Gassville Nursing Home on September 29, 2024.  She was born on February 14, 1936, in Chicago, Illinois, the daughter of John and Julia (Ryan) Small. 

Phyllis was a loving wife of Richard Sullivan and devoted mother to Jerry, Danny, Thomas and Cathy.  She was a registered nurse by trade and enjoyed her job of helping people.  She lived in Hometown, Illinois for 62 years and then moved to Arkansas in 2020 so her son and his wife could help take care of her.  She was an avid crafter and crocheter.  Playing cards and board games was what sparked Phyllis’s interest.  This would occupy her for hours. 

Phyllis is preceded in death by her parents John and Julia Small, two children Thomas and Cathy, and two sisters Patsy McGurie (late John) and Helen Benson (late Joe). 

She is survived by her two sons, Jerry (Donna) and Danny Sullivan, brothers Robert Small (late Mary Jean), John (Nancy) Small, Ed (Debbie) Small, Richard (Maria) Small, Micheal (Dorothy) Small and sister Mary Polito (late Phil).  Also, Phyllis leaves behind numerous nieces and nephews. 

Cremation arrangements are under the care of Diamond State Cremation in Lead Hill, Arkansas.
